What is auditory neuropathy?

Auditory neuropathy is a type of hearing disorder where sound enters the ear normally, but the transmission of signals from the inner ear (cochlea) to the brain is impaired. This condition affects the ability of the auditory nerve to effectively transmit sound information, leading to difficulties in processing and interpreting sounds, especially speech. Unlike more common types of hearing loss that result from damage to the hair cells in the cochlea, auditory neuropathy involves a disconnect in the way sound is processed by the auditory nerve or in the brain’s ability to decode the signals.

  • Causes: Auditory neuropathy can be caused by various factors, including damage to the auditory nerve, issues with the synapses between the hair cells and the auditory nerve, or neurological disorders that affect the auditory pathways in the brain. It can be congenital (present at birth) due to genetic factors or develop later in life due to conditions such as auditory nerve damage from ototoxic drugs, infections, or other neurological conditions.
  • Symptoms: Individuals with auditory neuropathy may have normal hearing thresholds, meaning they can detect sounds at normal levels, but they struggle to understand speech, particularly in noisy environments. They might experience fluctuating hearing ability, where their hearing seems better at some times than at others. Tinnitus (ringing in the ears) is also a common symptom.
  • Diagnosis: Auditory neuropathy is typically diagnosed through a combination of tests, including auditory brainstem response (ABR) testing, which measures the electrical activity in the auditory nerve and brainstem, and otoacoustic emissions (OAE) testing, which assesses the function of the hair cells in the cochlea.
